Provides a comprehensive, coordinated service system for older and disabled individuals in the nine county planning and service area ensuring availability of a variety of services in their communities to help them secure and maintain maximum independence and dignity in a home environment. Administers the Community Care Coordination Program, Family Caregiver Support Program, Housing/Home Repair Program, Information and Referral, Long-Term CareOmbudsman Program, Housing Directory for Seniors, Energy Assistance, HEMEN (Helping Elderly Meet Essential Needs), Training, Alzheimer's and Caregiver Respite, Assisted Living Program and PASSPORT Program. Case management of Ohio Homecare waiver individuals. Covers nine counties: Ashland, Crawford, Huron, Knox, Marion, Morrow, Richland, Seneca, and Wyandot.
