Description

People who are three years of age and older and are enrolled with Riverside are connected with a Service and Support Administrator (SSA), who meets with each person and their team to discuss what is important to them and their dreams for the future. Outcome-based Person-Centered and Self-Directed Plans are developed. The SSA serves as the primary point of coordination to develop, coordinate, and implement the plan, working with each person, family, and provider to ensure service delivery.
