Serves ages 6 months and older including municipalities located outside the city of Cleveand.

E-mail address Photo ID proof of employment in a Group 1A eligible position such as a work badge or letter from employer. A completed registration form (available on the website) will be required at the time of the vaccination appointment.

Application process

Those who qualify for Groups 1A and 1B (1B individuals must be 80 years old and older)should visit the website and use the link which states "Please click here to provide your contact information" and follow the steps on screen to verify eligibility and complete the sbmission of their contact information. Once this has been completed the individual will be notified (by e-mail and by phone) when new vaccination appointments are available. The individual can follow the link in the e-mail to see a list of available vaccination appointments and can then choose one of the slots. After completing registration, the individual will receive an e-mail confirmation. At the time of the vaccination appointment the individual should bring a completed registration form (available on the website).

Includes Environmental Health Division (food protection, risk assessments, septic system and well safety, solid waste disposal), Prevention and Wellness Division(nutrition services, wellness programs, dental care referrals), immunizations, lead poisoning prevention and detection, communicable disease prevention and control, child care education, school health, etc.), and Epidemiology/Surveillance Division. Acts as liaison with Early Start and Bureau for Children with Medical Handicaps.
