- Pathways HUB is available Monday through Friday from 8:00am to 5:00pm. - Services are community based and location and time of meetings are determined between individual and their CHW.

Description

Provides adults or children who are living with a chronic medical condition or at risk for a chronic medical condition such as diabetes, high blood pressure, asthma, cancer, and so forth with a Certified Community Health Worker (CHW). CHWs provide care coordination, social services support, and services focused on the social determinants of health in order to improve health outcomes. The CHW will provide assistance with connecting primary and preventative care.
