Application process

Application required. Applications may be completed online. Other required documentation may be dropped off in person at the County Health Center or the NKY Health District Office.

Note: There may be a waiting list for services based on the number of applications received.

Eligibility

Open to individuals age 18 and older who:

  • have been a resident of Boone, Campbell, or Kenton County for at least 60 days
  • have no insurance or limited insurance that covers dental services
  • have a household income at or below the limits listed online
