Application process
At least 6 weeks before trip:
  • Complete and submit the Traveler History Form at www.loraincountyhealth.com/vaccines or call the Vaccine Appointments line to have the form mailed to you.
  • A health departemt nurse will call to review your travel plans and health history with you.
  • The nurse will schedule your travel health appointment.

  • Varies according to vaccine and insurance plan, if applicable.
  • Payment is required on the day of service.
  • Accepts cash, check Mastercard and Visa.
  • A receipt for insurance reimbursement purposes is available upon request.

Provides inoculations to prevent individuals who will be traveling outside the United States from contracting infectious diseases such as typhoid, typhus, hepatitis A and B, malaria, dysentery or yellow fever. Available travel vaccines include:
