Application process
Walk-in (Monday - Friday) for a 90-minute behavioral health diagnostic assessment, or call to schedule an appointment. New primary care or infectious disease patients do not require a diagnostic assessment to establish care.
Fee
Medicaid and Medicare insurance accepted. Sliding fee scale program available for uninsured or private insurance, based on household income. Does not process private insurance claims except for some grant-support sexual and reproductive health services.
Eligibility
No restrictions for in person care. Must be in Ohio to receive telehealth services.
