Eligibility
- Serves caregivers with legal custody or guardianship of a child whose birth parents cannot care for him/her.
- Child and caregiver must live in Portage County.
- Family (including child) must have income no greater than 300% of the Federal Poverty Guideline Level.
- Custody must have been awarded on or after July 1st, 2005.
Required documents
- Verification of legal custody or guardianship.
- Proof of income.
- If needed, Agency will help clients pay for any required documentation that the client does not already have.
