You do not need to bring any documents with you, but you will be required to complete and sign an application. A birth application requires the name of the person on the birth certificate, date of birth, and mother's name. A death application required the deceased's name and date of death.

We will make arrangements for interpreters, if necessary.
We only issue certified copies of birth and death records. The cost is $22.00 each. The distribution of this fee is as follows: $8.50 is retained by the Health District, $9.00 is sent to the Ohio Department of Health, $1.50 is sent to the Ohio Family Violence Prevention Fund at the Ohio Department of Public Safety, and $3.00 is sent to the Ohio Children's Trust Fund at the Ohio Department of Job & Family Services.
