Determined at the time of intake.

Required documents

-Identification, -Insurance information, -Social security number of patient, -Current medication list, and -Custody or guardianship documentation (if applicable). If you are uninsured, -Proof of income, and -Proof of residency (can be a piece of government-issued mail such as a cell phone bill, utility bill, bank statement, etc.).

Hours

Monday - Thursday: 8:00 am to 6:00 pm. Friday: 8:00 am to 5:00 pm. Walk in hours: Monday - Friday: 8:00 am to 2:00 pm.

Spanish. Sign Language. Other languages through Language Line.

Walk in assessments available. Specific appointments can be made by calling the location.

Sliding scale available. Medicaid, Medicare, and most private insurance accepted.
