Birth to 22 years of age. Attached to 2 more agencies (school issues, multiple doctors, mental health, etc.)

Referral Packet, Release of Information based on program additional information may be needed.
Hours
Coordinator is part time- hours vary based on need.
Application process
Call 740-684-4085 or visit website at www.fcf.ohio.gov/Contact-Us/Local-FCF-Perry for information. Referrals typically come through provider agencies, but any family may self-refer for service coordination. A referral must be submitted, then a CANS assessment will follow. This determines what services can be offered based on need (not income). FCFC Service Coordination /Wraparound, Multi System Youth, Kinship Care Connections, and Local Fubds are utilized.

Perry County, Ohio
Assists in developing a collaborative plan of action for multi-need children and adolescents upon referral. A child may be considered a multi-need child if he/she is involved or eligible for involvement with more than one FCF involved agency (BDD, Juvenile Court, mental health or substance abuse services, among others.) Secondary services such as peer support partners, respite care, intensive home based services, in home support services may be available to children that are active in the Wraparound program.
