Serves homeowners in Summit County. Must be current on homeowner's insurance and property taxes and meet income guidelines. Home must be owner-occupied.
- Completed application. - Name, address, phone number, and dates of employment with all employers for the past year. - Proof of identity (photo ID, birth certificate, or social security card). - Copy of pension stating monthly or yearly earnings. - SSI and SSDI award letters. - Copy of complete divorce documents/decree. - Copy of bankruptcy discharge (only if occurring before the five (5) year limit). - Copies of last six (6) months bank checking/savings account statements. - If any adult in the household is a full time student the class schedule must be provided. - Copy of homeowner's insurance.
