Income must be at or below 250% of the federal poverty level; Have had a recent eye exam (prescription dated within the past 24 months); have no other resources available to pay for glasses, including federal or state programs or assistance from local charitable organizations - as this is a program of last resort. Applicants are eligible for a voucher once in a 12-month period.
Must provide proof of need- SNAP card or letter, Tax return, pay stub, Food Stamps card or letter, etc.
M-F: 9:00am-4:00pm.
Application must be submitted through agency website. Application can be submitted by a social service agency, school nurse, or by the individual. See website for more details.
Fixed fee ($10 application fee for all applicants.)
