Open to Medicaid recipients age 18 and older who have Humana as their State Medicaid Managed Care Organization (MCO). Trailer Park/lot must be the enrollee's permanent residence to be eligible for assistance.
Note: Enrollees who reside in a residential or nursing facility are not eligible.
