Open to individuals who left the Ohio foster care system at age 18, 19, or 20, but have not yet turned 21 and meet at least one of the following criteria:

  • Completing high school or GED
  • Enrolled in college or vocational school
  • Participating in a program or activity designed to promote, or remove barriers, to employment
  • Employed at least 80 hours a month
  • Incapable of doing any of the above activities due to a physical or mental health condition
Proof of income (Paystubs from previous 30 days, if applicable),Other documents (Enrollment letter, class schedule, attendance report, or grade report if completing High School or GED or attending higher education, Attendance/participation report from previous 30 days if participating in a program or activity designed to promote, or remove barriers to, employment, Copy of Social Security award letter effective within the past 180 days if qualifying based on physical or mental health condition)

Description

Offers assistance with the transition from the child welfare system to adulthood for eligible youth who left foster care in Ohio. Participants will be assigned a liaison who can help them to locate safe and stable housing, enroll in higher education, find a job, connect with community resources, and develop other independent living skills.
