Complete the Victim Notification form found on the website and fax or mail to our office. On receipt of the notification form, the request is processed and a confirmation letter is mailed to the requestor

The Office of Victim Services offers many services to crime victims, including, but not limited to, crisis intervention and advocacy throughout the corrections process, assists victims with concerns related to inmates and parolees under our jurisdiction, provides Information regarding status of inmates in prison or under supervision of the Department's Adult Parole Authority.
