Individuals with a severe mental illness who live in Geauga County and are U.S. citizens. Individuals who are not already on Medicaid must be referred by their mental health provider. Those already on medicaid should contact their caseworkers. Financial eligibility is an income at 225% Federal Poverty Level (FPL) and assets up to $2,000 for an individual, $3,000 for a couple. Some individuals may also be subject to asset limitations. Must have been diagnosed with a severe mental illness, or have been diagnosed with one of the following chronic conditions: such as Cancer, HIV/AIDS or Immune Deficiencies, End Stage Renal Disease, Sickle Cell Anemia, Cystic Fibrosis, Hemophilia. Also serves those who have had a previous transplant as well as those who are active on the solid organ or soft tissue transplant waiting list. Those with chronic conditions must also be over age 21, receive Social Security disability benefits, have one or more inpatient admissions due to the chronic condition, receive a qualifying score on the Adult Needs and Strengths assessment, and reside in an approved Home and Community Based Services setting.
Agency can look up most supporting documents through an electronic records search - the client will be notified if they need to supply any extra supporting documents. If necessary, client may be asked to provide one or more of the following: proof of age, medical proof of disability, proof of medical expenses, proof of income and assets. Applicants who are U.S. citizens and not receiving Medicare or SSI may have to show proof of citizenship. Primary documents for this purpose are a valid or expired passport, certificate of naturalization, or certificate of US citizenship those who do not have one of these will need TWO documents: a certified birth certificate, hospital birth record, or religious birth record made within 3 months of birth, PLUS an identity document, which in most cases is a government issued ID such as state ID or driver's license. They may also need documentation of name change for any reason. If client cannot afford the necessary documents, or documents are unavailable, agency worker will assist the client to obtain them or find other options. Services will NOT be denied or terminated because of this requirement, as long as client makes an effort to comply.

Clients may begin the application process, renew, and/or update their case information by calling the Eligibility Contact Center: M-F: 8:00am-4:30pm. May also apply, renew, and/or update case information through website anytime (client must have an email address to do this). No interview is needed. Agency can look up most supporting documents through an electronic records search - the client will be sent a checklist for any missing supporting documents. Once an eligibility determination is made, a notice of action (NOA) is mailed to the applicant. Applications are available and may be dropped off M-F: 8:00am-4:30pm MCP-eligible clients must select and enroll in an MCP, or will be auto enrolled randomly (plan can be changed with reason, or during open enrollment). Those with general questions about Medicaid, Medicaid Managed Care Plans, or who simply need to change their address/contact information can call the Medicaid Customer Service Hotline M-F: 7:00am-8:00pm; Sa: 8:00am-5:00pm. Starting March 28th, 2025: Clients using the online benefits portal will need to either create or sign into an OHID account. After they have created their OHID and logged into it, they will be asked to set up Multi-Factor Verification (a security feature to protect against identification theft). Those who need help navigating the set up process can phone the Contact Center M-F: 8:00am-4:00pm.

Enrollment is free, there may be small co-payments for some services.

Provides public insurance to cover medical services that may include doctor visits, some prescription drugs, hospital care, home care, long-term care, medical equipment, transportation, mental health, vision, dental, substance abuse services and other services. Most clients will have to choose and enroll in (will have their care provided by) a Managed Care Plan (MCP). When it is time to renew, Enrollees will receive a packet, including information regarding Medicaid status and forms for re-certifying coverage.
