Individuals who cannot work because of a physical or mental condition that's expected to last at least one year or result in death AND who meet the minimum length of employment requirements (or whose spouse or parents meet the requirement). Certain Social Security programs allow individuals on SSD to perform limited work and maintain their cash and medical benefits.
Social Security card and/or number, driver's license or other picture ID, most recent W2, account number and routing number for direct deposit and birth certificate. Applicants whose information is not already in the system will have to prove their U.S. citizenship or immigration status. Applications may be processed faster if provide medical records, list of health care providers and employer/job duties from last 15 years.
Local offices are open to the public M-F: 9:00am-4:00pm. The 800 number operates live M-F: 8:00am-7:00pm and with recorded information after hours. Many questions can be answered using this option.

Description

Provides monthly cash benefits to disabled or blind persons who meet minimum length of employment requirements (or whose spouse or parents meet requirement). Benefit amount is based on worker's earnings history. Those who receive SSDI for 24 months are eligible for Medicare. SSDI recipients with limited income and assets may also qualify for SSI benefits.

Also administers the assignment of Social Security numbers, changes in records, and processing of reports of lost or stolen checks. Individual Social Security offices are responsible for applicants who live within their service area, and all services are carried out at the local level. Clients are encouraged to go to the office which serves their Zip Code, though this is not mandatory.
