Case Management / Independent Living Skills - Vision Loss | Cleveland Sight Center

Provides instruction in client's home in a variety of practical skills for safe and independent living. May include safe food preparation, cleaning techniques, clothing care, money management, budgeting, shopping, communication (Braille, keyboards, and telephone use) and orientation. Also offers case management to connect the client to necessary community services. Program is tailored to individual client's needs.

M-F: 8:30am-5:00pm, but some accommodation to client schedule is possible occasionally.

Application process

Phone for registration. An intake associate will take all information and request an eye exam report dated within 6 months prior to application. After report is received, client will be registered and assigned to a caseworker to arrange service.

Fee

Sliding fee scale - no one is denied service because of inability to pay.

Eligibility

Open to anyone who has an eye condition that limits functioning, and desires this type of training in the home. Serves Cuyahoga, Lake, Geauga, and part of Medina County.
