EFFECTIVE IN JANUARY OF 2011: The Richland Public Health office of Vital Statistics can now issue birth certificates for ANYONE born in Ohio. Records and issues birth and death certificates in Richland County from December 20, 1909 to present. Records for persons born or died in Richland County from 1867 to December 19, 1909 are held at the Richland County Probate Court, 50 Park Avenue East, Mansfield, OH 44902 (419) 774-5583. Our office can only issue Death certificates for deaths occurring in Richland County, excluding the City of Shelby. Records for person who died within Shelby city limits after 1909 are recorded and issued at the Shelby City Hall, 43 W. Main Street, Shelby, OH 44875 (419) 347-5131.

Application process

Walk in to fill out form or visit https://richlandhealth.org/personal/birth-death-certificates/ to print off form, fill out and mail in for either birth or death certificates or complete and submit online.

Fee

$25 per certified copy for birth or death certificate. There is also a $1.55 convenience fee for using a credit card to make purchase.

Eligibility

Can issue birth certificates for any person born in Ohio. Persons that have died in Richland County on or after December 20, 1909--except those who died in the City of Shelby, Ohio after 1909.
