Required documents
Photo I.D. (For individual AND his or her representative, if disabled.) Proof of residency and one item of proof for each child claimed (such as Medicaid card, school record, etc.) needed on first visit. Residency information will be updated once a year.
Hours
By appointment: M, W, F: 9:00am-1:00pm. Closed on major holidays including Thanksgiving, day after Thanksgiving, Christmas, New Year's Day, Good Friday, Easter, Memorial Day, Fourth of July, and Labor Day. Also may close for inclement weather.
