Required documents
Proof of income (pay stub, W-2, tax filings). Photo ID to assist with Medicaid/Medicare eligibility process. Bring current insurance cards with you to appointment.

Mon-Fri 8 am-5 pm.
Languages
Spanish. Other translation services available.
Application process
Call (614) 274-1455 option 1 to become a patient/schedule an appointment at any of the seven Central Ohio locations. Patients are seen by appointment only and will be screened for COVID-19 symptoms prior to their appointment.
Fees
Medicaid, Medicare, commercial insurance and private pay are accepted. Sliding fee scale for uninsured. $10 minimum for sliding fee scale.

Provides ongoing primary care, sports and work physicals, childhood immunizations, annual exams for men and women, disease and condition screenings, prenatal care and sick care for adults, children and infants. Dental, Nutrition, Spiritual, Substance Use Disorder Treatment, Behavioral Health, Vision and Pharmacy care are available for Primary Care patients. Referrals can be made for specialty care and specialty pharmacy.
