Eligibility
Eligibility by under-performing school is determined by attendance and/or assignment to a public school building that has letter grades of D and F on their report card. Eligibility for the expansion of Edchoice is for families under 200% of poverty with students entering grades K-4.
Required documents
Completed application, proof of address and copy of student's birth certificate. If applying for low income status, W-2 or government benefit statements will be collected.

Application process
Students must apply and be accepted to a participating private school. The school will submit an application on behalf of the parent. A list of schools is available on the ODE website.
Fees
Application fees vary by school. Some schools may waive fee or accept a payment plan.
