Patient must be under the care of a physician and have his or her written approval, must be homebound due to a medical condition that prevents leaving the place of residence on a regular basis, and must be willing to participate in the treatment plan and cooperate with the health care personnel.
Proof of Insurance if applicable proof of income and family size if sliding scale is desired.
Office hours M-F: 8:00am-4:30pm. After hours there is a registered nurse on call for referrals. After a nurse is assigned to a case, patient has access to nursing assistance 7days a week.

Description

Provides both skilled and supportive services to homebound individuals who are under the care of a physician. A team of health care professionals, including nurses, nurse aides, social workers and dietitians, as well as physical, occupational and speech therapists, will work with the personal physician to help restore, promote, or maintain a person's health. Such assistance helps prevent the need for a trip to the emergency room or admission to the hospital.
