*Application available online or in person at the Fairfield County Health Department. *Applications can be mailed with a check made payable to the Fairfield County Health Department for $32.00. *Certificates may be ordered over the phone with a credit card.
Fees
$32.00 Fee. Will accept cash, money orders, credit card/debit card, and personal checks if the person's address and phone number are on the check.
