Offers individualized case management, mental health treatment services, social activities and planning, counseling, and advocacy for people with chronic mental illness, cognitive disabilities, and mood disorders. Life care planning may be provided if needed.

Annual membership fee, a one-time joining fee and fixed fees for services. Third party payment accepted: Medicaid, Medicare, Private insurance and private pay (however, these do not cover membership fee, joining fee or co-pay).

Eligibility

Adults 18 and older with a mental illness, cognitive disability, or an Autism spectrum disorder and lives in Cuyahoga, Geauga, Lake, Summit Counties.
