Annual membership fee, a one-time joining fee and fixed fees for services. Third party payment accepted: Medicaid, Medicare, Private insurance and private pay (however, these do not cover membership fee, joining fee or co-pay).
Eligibility
Adults 18 and older with a mental illness, cognitive disability, or an Autism spectrum disorder and lives in Cuyahoga, Geauga, Lake, Summit Counties.
