Eligibility
HMG Early Intervention-Infants and toddlers age birth to 3 who have a developmental delay or diagnosed condition as defined by rule.
Required documents
Will be requested as needed.
Hours
Mon-Fri 8am-4:30pm. After hours home visits are available based on needs of family
Languages
American Sign Language; Spanish.
Application process

Individualized supports include:
  • Comprehensive Developmental Evaluation
  • Connection to therapeutic services
  • Specialized services based on a child's individual needs
  • Assistance in transitioning to school-based or other programs.
  • IFSP - Individualized Family Service Plan
