Provides strucutred program to improve social functioning by identifying subjective non-medical social determinants of health, establish objectives to address identified areas, and case management support to develop, implement, and monitor a plan of action. Objectives may include but aren't limited to needed documentation, basic needs assistance, employment, budgeting, identifying unresolved legal issues, and finding community support groups.
