Certified copy of a birth certificate for those who were born anywhere in Ohio, can be ordered in person from the Madison County Health Department or upon receipt of a signed application or letter and the statutory fee. Death certificates are only available for a death that occurred in Madison County. Anyone may apply for a certificate, no ID is required. 740-852-3065 ext 1528
