Eligibility
- Must be 18 years or younger
- Family income must be at or below 200% of the Federal Poverty Guidelines
- Cannot have any kind of vision insurance
- Have not received a gift certificate through a Vision Service Plan (VSP) program within the last year

Application process
Complete application and submit to Holmes County General Health District, who will forward application to Prevent Blindness Ohio. Any applications mailed directly to Prevent Blindness Ohio will be thrown out and not processed.
