Adults age 18 and older. Indigent clients must reside in Cuyahoga County.

Medicaid or insurance information if applicable. Proof of residency such as driver's license/ state ID, current utility bill, lease agreement, piece of mail with current address, current election card, automobile registration, pay stub or recent tax return. Additional Information will be given during individual's phone contact with agency.

Office is open M-F: 9:00am-4:00pm, but evening services are also available. Patients attend treatment services at least 4 days a week, for a minimum of 12 hours a week.
