Application process

Contact the office by phone to receive quarantine and testing instructions. Then, download and complete an Animal Bite Information Form and return by fax.

Eligibility

Accepts reports from residents of Boone, Campbell, Grant, and Kenton counties who believe they may have contracted rabies or have come in contact with an animal that may be infected with rabies.
