All potential new patients need to complete new patient forms and turn in to Dental Front Desk. Once completed, patient must call to schedule an appointment. For Patient Transportation Service, call 419-522-6191.

All insurances accepted including Medicaid. Sliding fee is available for qualifying patients. Sliding fee means the bill may be reduced, not free. Please be prepared to make the minimum payment required at every visit.

Description

Provides dental care including exams, x-rays, cleanings, sealants, fillings, extractions, root canals, crowns, and bridges for adults and children. **NOTE**Each dental location offers emergency walk-in hours. Emergency walk-ins are meant for those experiencing extreme dental pain, swelling, fever, or signs of infection. These exams are problem focused and definitive care is not guaranteed.
