Individual must have an intake prior to receiving psychiatry services.
-Identification,
-Insurance information,
-Social security number of patient,
-Current medication list, and
-Custody or guardianship documentation (if applicable).
If you are uninsured,
-Proof of income, and
-Proof of residency (can be a piece of government-issued mail such as a cell phone bill, utility bill, bank statement, etc.).
Monday - Thursday: 8:00 am to 6:00 pm.
Friday: 8:00 am to 5:00 pm.
Walk in assessments Monday - Friday: 8:00 am to 2:00 pm.
