Open to residents of Clermont County age 60 and older who require assistance with certain activities of daily living. Eligibility for specific services is determined by a care manager during an in-home assessment.

Offers assessment and case management for older adults to assist them with remaining in their home as long as possible. Through case management services, participants have access to a variety of in-home services, such as assistance with chores, home delivered meals, personal care assistance, and application assistance for benefit programs such as food stamps (SNAP), Medicaid, and housing.
