Eligibility

- Serves residents of Columbiana, Jefferson and Portage counties. - Individual programs offered by the pharmaceutical companies have their own individual criteria.

Required documents

- Proof of income. - Driver's license or State ID AND Social Security card. - Medication bottles. - Prescriptions and/or samples from your physician. - All physician names, addresses, and phone numbers. - Current health insurance cards.
