FAMILY SUPPORTS PROGRAM | CUYAHOGA COUNTY BOARD OF DEVELOPMENTAL DISABILITIES

Provides funding for services/supports to persons eligible for Board of Developmental Disabilities (DD) services who live at home with their families or on their own. Types of eligible supports include camp assistance, home modifications, recreation, special equipment, specialized nutrition, and incontinence supplies.

Phone, email, or see website for information M-F: 8:00am-4:30pm. Once deemed eligible for DD services, applicant will then fill out an online application form for assistance.

Eligibility

Serves Cuyahoga County residents who are eligible for Board of DD services. Individuals living in an Intermediate Care Facility, in foster care, or enrolled on a Medicaid and Home Community Based waiver would not be eligible. Generally, serves those whose developmental disability occurred before the age of 22. All eligible individuals are eligible for some funding regardless of income. Taxable income is used to determine the total amount of funding provided for individuals under the age of 18.
