Case Management/Service Coordination | Summit County Developmental Disabilities Board

Using a person-centered approach to identify the supports an individual needs to meet their desired outcomes, staff works with the individual to select paid disability service provider or unpaid support to meet those outcomes. Coordinates the budget and funding for those services and monitors the quality of those services.
