Serves residents of Lorain County.
- Driver’s license, state ID, passport, or military ID. - Medical or insurance card, if applicable. - If have no insurance: Proof of current income, including income of any spouse. Proof of income includes pay stubs and benefit statements. - Custody paperwork will be required for a child to be seen unless both parents come. If one is a single mother, never married with no custody paperwork filed with the courts there will be no custody paperwork needed. If bringing multiple children: Each child and the legal guardian will go through the process individually, they will each have their own session, and the legal guardian must be present for each session. - If legal guardian of an adult, guardianship paperwork will be required.
- Clients may walk in for an assessment at the main office located at 6140 South Broadway in Lorain (44053) Monday through Thursday from 8:00am to 2:00pm, Friday from 8:00am to 1:00pm (excluding holidays). - Counseling services are by appointment. See site hours for holiday closures.

- Video Remote Interpreting and Telephone Interpreting services available in over 250 languages. - If a situation arises where an in-person interpreter is required, Riveon Mental Health and Recovery makes every possible effort to secure an in-person interpreter. - Printed programmatic information is available in Spanish.

- Call for an appointment at the Sheffield Village location. - Clients may walk in for an assessment at the main office located at 6140 South Broadway in Lorain (44053) Monday through Thursday from 8:00am to 2:00pm, Friday from 8:00am to 1:00pm. - Clients should plan to be there for at least two (2) hours for initial visit. - If bringing a child – Child(ren) must be accompanied by their legal guardian who must stay with the child for the entire session. Custody paperwork will be required for a child to be seen unless both parents come. - Sliding fee scale available for Lorain County residents. - Accepts most insurance plans and public programs including Medicaid, Medicare, and Title XX. NOTE: Client may be responsible for all or a portion of the cost of the assessment and services depending on their insurance.
