OHIO PASSPORT WAIVER - PASSPORT HOME CARE | DIRECTION HOME AKRON CANTON AREA AGENCY ON AGING AND DISABILITIES

Provides services to older adults in their homes as an alternative to nursing home care. PASSPORT services may include personal care/respite, homemaker, adult day care, home delivered meals, chore service, minor home modifications, adaptive devices, home medical equipment, pest control services, transportation, social work, counseling, emergency medical response systems, independent living assistance and community transition services.

Application process

Call the office for more information. An in-person assessment is conducted by a Registered Nurse in order to determine eligibility.

Fee

There are no charges for PASSPORT assessment, care management or home care services, although some higher income individuals may be required to make a monthly payment for a portion of their care as a condition of Medicaid eligibility.

Eligibility

Serves residents of Summit, Portage, Stark, and Wayne Counties age 60 and over. Must meet the following criteria: - Medicaid-eligible. - Need a skilled or intermediate level of care. - Living in a non-institutional setting. - Not receiving Medicare/Medicaid hospice care. - Needs cannot be met by other community resources. - Health and safety can be adequately met. - Client must agree to service plan. - Physician must agree to and sign the service plan.

Plans, funds, and coordinates services for older persons and persons with disabilities in Portage, Stark, Summit, and Wayne counties to live with dignity and choices in their homes and communities for as long as possible. Administers PASSPORT, MyCare Ohio, The Assisted Living Waiver, The Ohio Home Care Waiver, Care Coordination, Elder Rights, Family Caregiver Support Program, Nursing Home Placement Assistance Program, and other services designed to help older adults and individuals with functional limitations maintain independence. Provides funds for services administered by other community agencies including client finding, specialized information and referral, transportation, home-delivered meals, medical treatment, personal care, respite care, safety monitoring, health assessments, congregate meals, recreational activities, and adult day care.
