Eligibility
Persons with a hearing problem living in Ashland, Huron, Knox, Crawford, Wayne, Morrow, Richland and surrounding counties within the greater north central Ohio region.
Required documents
Proof of identification, proof of insurance and proof of income when appropriate to determine need.

Referral may come from an individual, family member, family physician, care giver or employer. Schedule an appointment by calling The Rehab Center Audiology Services at 419-756-1133
Fees
Insurance/Medicaid/Medicare/Self-Pay/Sliding Fee Scale determined by income and number of members. Hearing aids and assistive listening devices are not part of adjusted fee. Pre-Authorization with an insurance company can be done prior to scheduling an appointment.

Comprehensive hearing evaluations for infants, children and adults. Diagnose degree and type of hearing loss for people of all ages. Hearing aid services include; selection, dispensing, fitting, repair, cleaning or hearing aids and assistive hearing devices.
