Age 55 or older and have been declared incompetent by the probate court. Does not accept those whose primary diagnosis is a mental health issue.
Required documents
Statement of Expert Evaluation confirming a substantial mental impairment as defined by law, completed by a licensed physician or licensed clinical psychologist, and must be obtained within 90 days.

Provides advocacy and management of individuals' finances, health issues, and other affairs when someone is deemed a ward of the state by a court by either volunteers or professional staff, depending on the complexity of the issues. Emergency guardianship is available if needed. Services provided may include assistance with health care and financial decisions, transportation to and from appointments and assistance with nursing home placement.
