Eligibility

A patient must:

  • have a confirmed diagnosis of a serious health condition or be getting testing for the condition
  • be in active treatment for the health condition, starting treatment in the next 60 days, or have finished treatment in the last 6 months
  • be a U.S. citizen or permanent resident and be receiving treatment in the U.S.

Application process

Call to begin intake process. A limited number of new cases are accepted per day. Individuals whose cases are not accepted at the time of their initial call may call back the next business day.

Provides case management to address issues with health insurance, disability benefits, and other financial hardships impacting access to care for individuals with a chronic and life-threatening medical diagnosis. Assistance offered may include eligibility screening for benefits, application support, benefit optimization, help addressing coverage-related issues, appeal support, and connection to financial support for medical debt and daily expenses.
