TAKE CHARGE OF YOUR HEALTH | FAIRHILL PARTNERS

Provides small-group interactive evidence-based workshops for older adults with one or more long-lasting health condition, and for caregivers. Participants set their own weekly goals as they learn and practice self-management skills to improve their self-care and quality of life. Workshops meet for six (6) weekly sessions and include: Chronic Disease Self-Management, Diabetes Self-Management, Chronic Pain Self-Management, and Building Better Caregivers. Phone, online, and in-person programs available.

- Serves individuals age 60 and over with a chronic illness and caregivers of individuals with a chronic illness residing in Cuyahoga, Geauga, Lake, Lorain, and Medina counties. - Serves individuals age 50 and under residing in Cuyahoga County only.
