Eligibility

Single women, aged 18 and older who have been verified HOMELESS by Cuyahoga County Coordinated Intake. Client must have an established acute medical condition or chronic medical condition needing stabilization. Must be receiving some form of treatment to manage/stabilize their condition. Must be independent with all activities of daily living (e.g., bathing, toileting, etc.). Client must be independent with mobility and with any assistive devices. Must be cognitively and/or behaviorally appropriate for group setting, and not presently exhibiting symptoms which require a higher level of care (i.e., actively suicidal and/or homicidal, frank delusions which impair functioning, acutely disorientated, wandering, etc.).

Application process

Intake for individuals is through Coordinated Intake at Bishop William Cosgrove ( use the taxonomy: EMERGENCY SHELTER CLEARINGHOUSES*SINGLE ADULTS). If discharging a patient, hospital staff may either call Coordinated Intake to arrange to have an outreach worker come to the facility to do an intake or can contact this agency directly. Individuals who have already been released will need to go directly to Coordinated Intake at Bishop William Cosgrove to self-refer (they should indicate that they are interested in being placed into Joseph & Mary's Home). Those who self-refer will need to have medical documentation of their current health condition - if they do not have this documentation, they must sign a release of information form so that the discharging facility can share that information. Individuals and medical professionals can call Joseph & Mary's Home directly for information and guidance on the intake process M-F: 8:00am-4:30pm, however, Coordinated Intake at Bishop William Cosgrove will need to be involved before this shelter can be accessed.

Description

Provides transitional shelter for homeless women who are recuperating from an illness or injury. Offers a facility with private rooms, ongoing medical supervision, medication management, meals, and coordination with local health care providers. Residents also receive intensive case management that includes submitting housing applications, benefits review, assistance to file for SSDI and to resolve barriers to self-sufficiency, healthcare enrollment, and rent/utility assistance. Provides on-sight substance abuse/mental health assessment and treatment, support groups, and transportation to and from any needed medical or housing appointments.
