Prescription Drug Patient Assistance Programs | Faith Community Pharmacy
Prepares, stores, and dispenses medication at no cost to those who unable to afford their prescriptions. At each visit, pharmacy staff fill a three-month supply of medication, provide individual medication counseling, and answer prescription-related questions. Patients with significant transportation barriers may be eligible to have their medication(s) delivered to their home.

Application process
A patient's doctor or healthcare provider must submit a completed referral form and their prescription(s). Referred individuals will then be contacted by phone to schedule an intake appointment. Individuals who do not have a regular doctor/healthcare provider may contact the pharmacy directly for assistance.

Eligibility
- Any resident of Boone, Campbell, Grant, or Kenton County in need of assistance affording their prescription medication is eligible to receive a 90-day supply of medication.
- Residents with a household income at or below 300% of the Federal Poverty Level are eligible for ongoing prescription assistance.
Faith Community Pharmacy serves all residents of Northern Kentucky regardless of immigration status.
