- Serves Ohio Direction Card (food stamp) recipients residing in Carroll County who have lost food due to a power outage, fire, or flood. - Loss of Electricity: Power must have been out for at least four (4) hours or more due to a situation beyond the client's control (a shut-off due to a lack of payment does not count). - Flooding or Other Disaster: Will only help with replacement due to flooding or other disaster if the incident is a governor declared emergency.
- Client will need to complete a "Statement Requesting Replacement of Supplemental Nutrition Assistance Program (SNAP) Benefits" form (JFS form 07222, Rev. 9/2023). - For loss of electricity: Households should be prepared to provide verification of the extended power outage. This includes screenshots of news reports, text or email alerts, or outage maps that show their specific area was without power for four (4) hours or more. Client may call their utility provider's customer service department to request written verification of power outage and its duration. - For fires: Client will need documentation from the Red Cross.

- Call to request application or walk in Monday through Friday from 7:45am to 4:30pm. - Client must return application to the agency (on site or by mail) within 10 days of the reported loss. Once Complete Application is Submitted: - A prorated credit will be added to the Direction Card when eligible applications are approved. To Obtain Verification of the Outage: - The client should call their utility provider's customer service department to request verification of the outage and its duration. Verification will then be mailed or faxed to the client.

Description

Provides replacement funds to Food Assistance (food stamp) recipients who have lost food due to a power outage, fire or governor declared emergency. Replacement funds may replace up to (but not exceed) the client's monthly allotment of food assistance. Funds are sent by electronic transfer to the recipient's Ohio Direction Card.
