Appointment required to apply. Appointments may be scheduled by phone.

Eligibility

Participants must: -Be a resident of Kentucky -Be a pregnant, breastfeeding or postpartum woman OR an infant/child up to 5 years old -Have a household income at or below 185 percent of the poverty level OR have a Kentucky medical card, receive Kentucky Transitional Assistance Program (K-TAP) or Supplemental Nutrition Assistance Program (SNAP) benefits, or have a pregnant woman or infant in the family who receives Medicaid or K-TAP Poverty guidelines can be viewed on the following website: https://aspe.hhs.gov/topics/poverty-economic-mobility/poverty-guidelines
