The MCHD’s policy for a client to receive Nursing Services is that you must present a photo ID and all insurance cards/medical cards(if insured). If the person seeking Nursing Services does not have medical coverage, a photo ID is still required. This ensures our accountability to those we serve and to the insurance companies we bill for services.

Immunizations are administered Mon-Fri 8am to 12pm and 1pm to 4pm.
Fees
PHN Services are offered regardless of the persons’ ability to pay. For more information, please call Leanne Cunningham, BSN, RN, DON, or Angie Rosler, BSN, RN at 740-992-6626 weekdays between 8am and 4pm.
