Required documents
Insurance cards must be presented at every visit. If patient would like to sign up for the sliding fee, proof of household income must be provided.

All insurances accepted including Medicare and Medicaid. Sliding fee is available for qualifying patients. Sliding fee means the bill may be reduced, not free. Please be prepared to make the minimum payment required at every visit.
